Survived my last night shift for a little while. Time to take care of myself.
I tried to escalate the rota gap. I emailed the clinical director, chief of medicine, MD, CEO and GOSW.
Its now been 24 hours. I've not even had a single reply or acknowledgement.
NHS culture.
Medical consultant sees patient and notes rapidly growing skin lesion on patients forehead. Suspects SCC. Refers to Derm for 2WW opinion. Referral rejected - advised needs to see GP and then they can refer. Surely a huge waste of everyone's time??
#MedTwitter
I urge doctors of all grades to join
@TheBMA
. Further pay cuts for consultants. An absolute insult of 2% for burnt out, overworked and underpaid JD's. a
A huge pay cut. The time for industrial action is now. Join your union.

I'm having one of the worst days in my NHS career today. But I've just seen
@MarkCavendish
levelled the TdF stage wins record and I'm oh so happy. Got me into cycling as a teenager with those first wins and I can't honestly say how much joy it's brought me since. So thank you!
I hope
@TheBMA
are taking notes from
@RMTunion
. And in fact all doctors in that case. This is what it takes to strike. If we want to improve conditions we have to lose 'public support' and will get bad things said about us on the news!
7th med reg night in 14 days tonight.
Physically and emotionally exhausted. Insane pressure on the medical team. Sickness tonight with no cover forthcoming.
Everything in my brain tells me this is not worth it for my own health and wellbeing. It's sad.
Wish me luck.
Hmmm. Only getting scored 12/20 on 'commitment to specialty' when you've worked in it for 2 years and passed the SCE already. Something a bit off on that. Upset that my QI was in cardiology (which I also presented at BCS..). Doesn't seem like a specialty that wants trainees eh.
@trishgreenhalgh
Even as doctors there has been a huge silence about the fact it is and always has been airborne transmission. The 'droplet' dogma disappeared but why we are not supporting FFP3 for all staff with Covid patients still baffles me.
@walsh_joe
This is also assuming everyone passes first time, and no one dual CCT's or develops special interests/subspecialties. This is the absolute minimum.
@foi_requests
Unfortunately as the medical reg with a rota gap overnight it may well be a them problem but ultimately it's a me problem when I have less staff overnight and the management team are tucked up in bed.
The life of a rotational doctor. My wife is rotating to
@NGHnhstrust
today. She was meant to get a pay schedule 8 weeks ago. Despite several emails, this has been ignored. (1/5)
I deleted my last ID badge post. I fully expected my 5 followers to make a joke about me not being a real doctor. I just wanted to show how silly ID badges can be - not start a big PA scope creep debate.
Someone clearly saw it and the trust have contacted me to fix the badge ๐
@Dr_Done_
Oh my. Physician associate doctor. This is what happens without any regulation. I'd bet patients don't know he isn't a doctor with a medicine degree.

@theipaper
Mate I had to organise cover for my own wedding day. I must have got the wrong end of the stick and didn't realise I was in charge!
@Thornton_health
Can be the most senior doctor overnight (medical registrar) but seen as 'just' a worker. No words. At least I've confirmed what NHS management really think of me.
This is an illegal deduction of her wages. It's typical of being a rotational doctor.
@BMA_JuniorDocs

Sorry. The day after doctors announce joint strikes your main concern is about employing robotic penguins to replace workers? I guess they'd be less likely to strike. Probably cost a hell of a lot though. I do wonder if there's any conflict of interest here!
Why should she go and do her on call shifts this month. She's not going to get paid for them currently due to HR. This is why morale is so low. This is why we are all leaving.
@NGHnhstrust
can I receive any reassurances this is going to be sorted and she is paid correctly? (4/5)
@DBater29508
Serious answer.
1. I genuinely want to make a difference to patients. Looking after sick patients and seeing them get better is fulfilling.
2. Inertia I guess.
Now, she is employed by a lead employer (
@StHK_LE
) - who have now said she won't be paid in April for any of her on calls. Just a basic salary. Because
@NGHnhstrust
haven't provided any pay schedule details despite chasing. (2/5)

I'm medical reg on call in ED today. It's unsafe. Unfair. Poor care being given. Staff trying their best but woefully underequipped.

Have we not learnt! Another salient point is that if the NHS is left to suffer this winter I'm seriously thinking (and I imagine many other healthcare professionals are the same) of an entirely different career path. Already enough emotional baggage.

ยฃ684 a year to park at work. Disproportionately effects staff who work OOH/rotate (JD's!) with limited public transport options. Outrageous.

The government has to come to the table. The DDRB is NOT independent. It needs reform. They won't listen to the RCN but they will have to listen to the BMA.

>6hr wait for a type a aortic dissection to be transferred to a surgical centre yesterday. How has it been let crumble this far. (I'm by no means blaming those just trying to do their job.. it's the government's deliberate fault.)
@NeilDotObrien
How do you 'hire some more GPs'? Young GPs are leaving in record numbers. It's not medical school places that are the problem is it... It's retention.
@Doctor_Hutch
An argument to count the number of riders up the road and count them back in? It's not even like it was a big group! What a mistake to make.
Back from a well earned ride after 12.5 hours on a Covid ward yesterday. (Which is a horrid place). Neighbours have friends round in the garden drinking etc.... I mean words don't even describe it.

Yes and not only a consultant orthopaedic surgeon but input from a consultant in elderly care medicine too through the orthogeriatrics MDT. Can be some of the most challenging patients to look after in the hospital from my experience as med reg!

@jtjbooth
I don't know... The 'F3 phenomenon' is becoming the F4, F5 etc etc phenomenon. And why not when you're paid the market rate. The longer you take out the less likely you are to return to training. (All subjective of course)
@AbdoPainHotline
This is the worst mnemonic I've come across on all the ambulances outside my hospital. 'Its the worst you've ever felt' - could no one be more imaginative? The obsession with sepsis distracts and I find often saying the patient assumes someone is at fault for it?
@danielgoyal
No more money. I emailed the chief of medicine, clinical director, CEO, MD and GOSW yesterday. We survived.
How many of those people have even acknowledged my email?
Zero.
If that doesn't tell you about a workplaces culture I'm not sure what does.
Big up the athletes at the Olympics championing mental health. Simone Biles, Tom Dumoulin to name just two. Mental health problems are far more widespread than many care to admit. Many people are barely treading water. They're my champions.

Does this chart show they make a profit of half a million pounds but instructors often don't get paid to instruct on their courses? ๐

The nurses union is rightly upset. But it just goes to show if you don't pass your strike ballot then you are powerless to fight for better T's and C's
@DrKidneyAsh
As a group of doctors we should not out up with it. We need at least annualised rostering and forward planning so we can plan our lives like the responsible adults we are. Not treated like kids until we're into our mid 30s, at the earliest.
@UKGastroDr
@RCPhysicians
There is a huge amount of anger. There are hundreds of good doctors struggling to progress in training whilst they see these misused roles coming in and taking parts of their jobs. They see the RCP seemingly willing to enable it.

I firmly believe annual leave shouldn't be cancelled, ever. If you need people to work on their annual leave day... Offer them a decent locum rate to come in for extra work. It just makes morale even worse than it is.

I have previously slept in a sleeping bag in the boot of my car as medical registrar during my break. We have 2 'on call beds' for all of the medical staff in a dgh. It's card access and despite trying my card doesn't access it. But also there is no lock.
Today's going to be rough at work. Med reg on take. 20+ outstanding to be seen (plus all the med patients stuck in ED cos no beds) plus cardiac arrest bleep and surgical referrals. Longest wait to be seen 9+ hours from referral. Ouch.

Maybe don't treat doctors in a way that they end up on strike. Wasn't that the reason for the 'independent' DDRB in the first place? Problem solved.
I also ran the medical teaching programme and set up a journal club in the specialty I'm applying for. Not sure what else I could have done! I think feedback other than 'im worried your poster/QI was in a different (but part of the job!) specialty would be somewhat appreciated.

Agreed. Members of the public couldn't believe an fy1 doctor is paid less than a barista. Ultimately everyone's monetary value is comparable. The time for being polite has long gone. That's what's got us in to this mess.
Glad to know it'll take 14 years after qualifying as a consultant to become a consultant now rather than 18 years working as a consultant to become a consultant. Wait what.

I am also an Easy Midlands doctor and will be voting to strike. This article shows a complete detachment from reality. The NHS is now fundamentally unsafe with huge numbers of doctor vacancies. Why? Because we can go get as good if not better living standards elsewhere, (1/2)

Lol my wife was put on nights and I was put on long day on call for our own wedding. Despite giving notice.
Had to fight our own corner and only got it off due to generosity of colleagues swapping shifts.
So this GP on BBC breakfast looking after '400 families'? How many actual patients is this? How plausible is it with GP numbers? I imagine it's a bit if an anomaly presented as the norm? Happy to be corrected. Just interested!
