@OcoRory @r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @TheStrokeAssoc @RCollEM In EEAST, we have introduced our own apprenticeship MSc in urgent care and pay band 8a when they qualify with prescribing. We have a dedicated directorate providing community urgent care to keep staff and provide a better service @EEAST_UrgCare

@OcoRory @Meghpai @Parody_RCGP @gmcuk Thank you Rory. Incidentally, the article was intended for a staff newsletter on International Paramedics' Day.

Seeing undifferentiated patient, assessing, making clinical decisions, creating a plan and prescribing as necessary unsupervised but with access to senior advice. No claim to be as expert as specialist.

@OcoRory I have a proper doctorate (not an honorary one on the basis of two undergraduate degrees) from a Russell Group university so I am entitled to use the title. I 'm happy to evidence any of my statements so if you have a challenge, let's have it.

@doctorsp194 @r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @TheStrokeAssoc @RCollEM Advanced practice training (MSc) leaves a competent paramedic or nurse working at registrar level. Arguable then, that being assessed and treated by them is better than the average FY1 doctor. Doctors should see complex cases and ACPs less so.

@doctorsp194 @r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @RCollEM I do NOT think anything of the sort about PAs and didn't even mention them. They are not registered HCPs (yet) and it's your profession that's promoting them (see RGP allowing them to become hon. fellows). T

@r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @TheStrokeAssoc @RCollEM Article fails to appreciate that some experienced and skilled EMTs are competent to deal with cardiac arrests and that the extra skills paramedics bring aren't evidence-based.

@talthegooner @AnnonMedic36970 What a sweeping statement! I personally don't agree with allied health professions being used to replace doctors, especially PAs who currently have no registration, but we need to take a sensible approach to risk and utilising doctors appropriately.

@kyletoc_o @OcoRory @r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @RCollEM @EEAST_UrgCare We are establishing specialist tier for the many unscheduled rather than emergency patients that call ambulances. If paramedics call 111/GPs, they wait for hours if answered at all. Need to pass over to ACPs to get them back dealing with emergencies.

Improvements with the other things you mention are coming to fruition. All ambulance services need to up their gain though.

Annoyingly, I've just seen that I have two profiles! My proper one is @SJB293 which clearly has my proper title and bio. Happy to carry any arguments over to be proper accounts as I'm going to delete this one.

Hi all you lovely Cambs & Peterbro ambulance clinicians! Call Before You Convey is now open 08:00 to 20:00. Where else can you get instant advice from an ED consultant or funny old man?

A great use of a doctor's time! I'm only speaking in terms of contract/scope.

@toast36742904 @AnnonMedic36970 @OcoRory I didn't really. I became an ACP because the freedom to do more for patients (scope), job satisfaction, science background so study interesting to me, better pay, better hours and too old for emergency care. Also has three of the four pillars of advanced practice.

@rheumipainmask @OcoRory @Meghpai @Parody_RCGP @gmcuk Are you deliberately failing to misunderstand here PS? I did NOT say I'm more qualified than GPs. My mentors are GPs because they have higher qualifications and more diverse experience. I know what I'm good (and bad) at!

@Meghpai @Parody_RCGP @gmcuk But I'm a proper doctor Meg and legally entitled to use the title. The GMC can't/wouldn't crack down on doctors using the title doctor, however, purporting to be a physician when not on the other hand...

There is no way I would compare a nurse or paramedic as being at the level of a SpR in a specialism, more generally emergency medicine or unscheduled care.

I will try to find employers evidence that an ACP shouls be able to work at registrar LEVEL as explained perviously, not to say as well trained and experienced.

@toast36742904 @AnnonMedic36970 @OcoRory How do you know I'm subpar? I may be very good. I agreed about 30 years ago with a GP friend & BASICS that he was a frustrated paramedic and me a frustrated doctor. Happily, we both overcome our frustrations to help people @SJB293

I bet you would be annoyed being asked to see a patient with a simple chest infection. If it's potentially complicated, I see a GP. If not, I'm happy to see a paramedic or nurse.

@TreforR Really? Are we talking about GP who are former consultants in dermatology? Isn't the normal pathway med school (touching on the subject), a 6/12 junior role in dermatology if chosen, CPD in pharmacology or part of FRCGP? Sorry if wrong and happy to be corrected?

@AnnonMedic36970 @OcoRory Nope. It sometimes appears on my credentials, which I change when I can (AccuRX etc), but I make it clear that I am an advanced paramedic and when receptionist make appointments for patients, I'm "Our paramedic, Simon."

@ClipboardUK @r1chardf1tzg3r1 @SteveBarclay @AACE_org @ParamedicsUK @wesstreeting @TheBMA @rcgp @TheBHF @TheStrokeAssoc @RCollEM @EEAST_UrgCare - that's precisely what we're doing to retain paramedics, give them a clinical career pathway and provide an appropriate service to the many patients who out of desperation dial 999 as they can't get GP appointments.
