Perhaps as a medical community over the years we have done society a huge disservice by leading everyone to believe that we have all the answers. The truth is, despite huge progress, we have very few of the answers.

I am extremely tired of NHS management bashing. Managers I have worked with have been some of the hardest working altruistic and skilled people I know. How the heck do we think the NHS responded so quickly to the pandemic? Clinicians need managers to implement stuff and..er..

GPs tend to be bashed for testing and bashed for not testing. Bashed for referring, and for not referring. Bashed for admitting, and for not admitting. GPs get bashed. Why don't we use the data to problem solve at a system level and not bash anyone? Grumpy

Describing healthcare as 'primary care work' or 'secondary care work' locks us into old ways of working, feuds and silos. Maybe we should think about it in terms of what patients need, how and where they need it most, then allocate resource accordingly.

Why is it in healthcare that we are quick to invest in interventions that cost vast amounts for tiny benefit but fail to invest tiny amounts for interventions that have vast benefit? Are we evidence-based or not?

I'm a pretty measured kind of gal, but I honestly think that the changes to the core GP contract in England are sufficient to break general practice as we know it. It is also likely to worsen the Inverse Care Law so further worsening outcomes in areas of deprivation.

Manage. Blanket criticism of NHS managers is lazy rhetoric and has to stop. It's no different to berating GPs in my book. Yes there are bad managers blah blah but there are also bad clinicians, bad teachers, bad politicians....

2022 will see me retire from mainstream general practice to pursue other projects. It seems odd and a bit sad that it ends like this but I have decided. I have many memories, some good, some bad, but in my heart I will always consider myself a GP. I hope my colleagues do too.

Need some better news so here's some. Hospital Covid bed occupancy is definitely reducing in Wales and the teams are seriously cracking on with boosters. Interesting surge in uptake of first doses too in younger age groups. Stick with it Wales and get vaccinated! 👊

I am really really worried about general practice. Great general practice is not just about triage and algorithms. It's about relationships and is essential for great primary care. We erode it at our peril. Too many colleagues forced to surrender their contracts 🙁

The backlash against the GP profession this week is really awful. I know things aren't great at the moment but instead of lashing out at human beings who in general are working their socks off and care deeply about their work, how about we debate how to address the problems ?

Minor illness is a retrospective diagnosis. Avoidable admissions are also judged retrospectively. I see huge assumptions and over-simplifications made about our system.

We have no shortage of people to deliver vaccines, many of whom are volunteering their own time 7 days a week, 12 hours a day. It's a huge logistical exercise run by good people who want nothing more than to vaccinate at 100 miles an hour for the benefit of the Welsh people

I quite like the Japanese concept of Ikigai when thinking about our purpose in life. I understand it to have 4 components. To be happy and content we should do something we are good at, something that is useful for the world, something we are paid for and something we love.

It's getting faster. The rate - limiting step is vaccine supply. The supply chain is complex for vaccines always, so can you imagine the task to deliver something totally novel less than a year into the pandemic.

In UK healthcare it's not that hard to 'engage' a lot of people with a good idea. It is much harder to move all parts of the system to make it happen. So often we give up as a result.

Right I am wading in. I joined general practice in the days of the grand old Red Book. Income was way lower...but staffing was linked to patient numbers. Morale was said to be low but partnership was still in demand and regarded as an honour

Personally I will be wearing a mask for a bit longer yet even if rules are relaxed until infection rates come down This isn't because I think vaccines don't work. Vaccines are like armour...protective, but some armour is more protective than others, and it depends on the weapon.
