To put things into context, if I get COVID my law job would expect me to isolate for ten days and would pay me sick leave. My hospital would expect me back in zero to five days and I would have no pay for missed shifts. Is it clear why health care workers are burned out?

Hospital admin comes down to deliver the daily bed count, looks at me sitting at the doc desk and goes: “Oh is there not an attending today?” Because running an ED without a doctor some how makes more sense than me being the doctor 🙃 Don’t worry I kindly introduced myself.

My pregnancy is very wanted and very miserable. Here’s a pic from me at work covered in puke stains bc sometimes while driving I start vomiting uncontrollably. Nobody who doesn’t want to be pregnant should be forced to stay pregnant 💫.

I've recently started practicing medicine in Virginia and learned that VA Medicaid has 800+ diagnoses it considers "preventable ER visits" for which reimbursements are significantly decreased. Highlights include DKA, heart failure, and newly diagnosed brain tumors.

Saw my husband (also em doc) briefly on our way to/fro shift. We stayed 6 ft apart. I was happy to see him home safe—then he told me to stay safe before I headed off. This is our new norm. But this is what we signed up, and we’re going to keep showing up for our patients.

I propose that any attending who says residents should do 27 hour shifts should be required to do a yearly 27 hour shift themselves, to keep them honest. This should include attendings with small children and/or chronic medical or mental health conditions, just like residents

Baffling narrative I keep hearing in the news: symptomatic person can’t get COVID19 testing thus doesn’t know if they should quarantine. Yes we need more testing! BUT your decision to quarantine should not depend on testing. If you’re symptomatic you should stay home regardless

It felt like all of my patients yesterday had COVID, regardless of what they were in the ED for. Probably because DC just reported a 98!!!% test positivity rate. A month ago this would have been unfathomable. Even a week ago I didn’t think we could do much worse than 85%.

MedTwitter has been quick to note that medical students are vulnerable. They are eager, but also unpaid, and lack employment protections. Third year medical students are particularly eager with match on the rise, promise of improved MPSE cannot be overstated.

Today reported @EinsteinMed school will allow students to see patients again. We have now OBTAINED AN EMAIL which appears to ask students to "volunteer." Their service would be noted in their transcripts. This *clearly coerces those who wish not to 👇

When you make your pregnancy announcement via NPR interview because you’re so sick of everything. My husband and I can work ED shifts while COVID+, go to the gym unmasked, dine indoors, but we’re in a state of emergency and he can’t accompany me to outpatient OB visits. 🤯

Did you know that @AmerAcadPeds recommends the federal Dept of Labor adopt std of 12 weeks paid leave for new parents but only recommends 6-8 weeks of paid leave for new parents in GME training? This notion that resident are 2nd class members of the work force knows no bounds.

ED docs—does the idea of discharging a bleeding patient with a non-viable partial molar pregnancy to the parking lot (where EMTALA continues to apply) with instructions to check back in when she’s hemorrhaging because of a state abortion ban absolutely blow your mind? It should.

Reminder that breast feeding is not accessible to everyone. Because of past medical issues there is a good chance I can’t breast feed. This never worried me. Until now, five weeks before my due date, amidst a national formula shortage 🤯🤯.

Thought it was very romantic of DC medical licensing board to send my husband and I our papers in one jointly addressed envelope, sort of like a fun wedding invite. Then I opened it and saw the $1700 invoice. Suddenly much less romantic. Worst date night ever.

This is absolutely the opposite of everything we have been taught. There is no emergency in a pandemic. You do not rush in without PPE. Your other patients can not afford you to take that risk. @RamiAhmedDO
