Dear Hospital Consultant, when you give a patient an awful diagnosis, please pick up the phone&tell us (their GP), because it’s the first place they go afterwards. If we have no clue as to what is going on, it makes for a terrible consultation. Letters often take weeks to arrive.

Young healthy fully vaccinated, (incl booster), female referred yesterday for PCR. Mild flu like illness, antigen +ve. Symptomatic x 18hrs prior to referral. Got notification of PCR test today for Tues next Jan 4th by which time as per guidelines, she no longer needs to isolate🤷‍♂️

Hi @SimonHarrisTD I’ve a patient with probable Motor Neurone Disease. Needs test called EMG/nerve conduction study to confirm diagnosis & get on treatment to prolong life. Waiting time on “urgent” public list is six months. He/she may well be bedbound by then. Any suggestions?

3,000 doctors have left Ireland in the past 5 years to work abroad. About 30% of GPs are due to retire in the next five years. This is why people can't find new GPs or get same day appointments. There are just far more patients needing care than there are doctors to see them.

If symptomatic, cannot access a prompt PCR test, but have a +ve Antigen test, then you almost certainly have COVID, and need to behave as such. If Antigen test is -ve with symptoms, this doesn’t constitute sufficient grounds to definitively outrule COVID. PCR is needed.

Our practice alone today ordered enough PCR tests to crash the system. This is not viable. Algorithms need to be reintroduced to prioritise testing for those who need it most. I would predict today that >50% of tests ordered by Irish GPs will not be done.

Massive problems afoot here. I spent last night going through patient lists & identified many who fit Cohorts 4 & 7 who “should” have been vaccinated by hospital services whose care they are under, but there is significant variation between hospitals in terms of doing this.🤷‍♂️🥺

A centralised IT system is urgently needed for GPs & GP staff to enable us to check vaccine status by inputting a patient’s PPS number. Otherwise the whole proposed GP led vaccine rollout of Cohorts 4 & 7 will be fraught with difficulty.

Hi @SimonHarrisTD I’ve a patient with probable Motor Neurone Disease. Needs test called EMG/nerve conduction study to confirm diagnosis & get on treatment to prolong life. Waiting time on “urgent” public list is six months. He/she may well be bedbound by then. Any suggestions?

Saw man sent from orthopaedic clinic today. Attended there with broken foot. Mentioned to them re hip pain. X-rayed & showed advanced arthritis. Advised see GP to be referred back to that same orthopaedic service, regarding hip. And people wonder why there’s a shortage of GPs.😡

Joel Matip is no world beater but he’s an underrated under appreciated player. So much better and more reliable than Lovren. Happily hold onto him as 4th choice CB for next season.

No one “doctor” is going to change the current situation at LFC. Our injury trend has its origins in the fact that 80% of same player base are still being expected to play the same high intensity dynamic pressing game, five years later. That is not viable. Especially in midfield.

The Irish Healthcare System, where a patient lucky enough to have requisite private health insurance can have their knee replaced or cataract repaired in a matter of weeks, by comparison to medical card / public patient who will be lucky to be done inside two years. It stinks.😡

Advising that healthy young people with normally functioning immune systems who are mildly symptomatic with positive antigen tests, have to proceed with PCR testing makes little sense to me. This will do little to relieve the demand for PCR testing. 🤷‍♂️🤯

“Forced into it”?!🤷‍♂️Makes it sound like an optional bonus extra if we choose to buy a CB. It’s an absolute necessity & failure to would be hugely negligent. Fact that we have yet to address our biggest deficiencies just over a week from the start of the season is mind boggling.
