Say what you will about modern Bethesda games, Starfield does let you make a truly gaunt old man. Multiple liver spot options, multiple balding hair options, customizable crows feet, honestly one of the best examples out there in the field.

Baldur's Gate 3 just barely lets you make a sufficiently gaunt old man, but it is passable. No way to tone down the life in the eyes, the voices are all relatively youthful, and sadly, there is a rippling physique under the armor.
