I didn’t work with Mr Omer Karim, but have known him for many years and that he is a highly accomplished surgeon and a good man. So am very pleased to see that his struggle to clear his name has reached its rightful conclusion.

Fluid intake - around 2.5 L per day (each 200ml more fluid reduces risk by 13%). Coffee, tea (and alcohol!) are PROTECTIVE.
But AVOID sweetened colas etc.
Lemon Juice and OJ increase urinary citrate but 2L needed

Agree totally
I always do semi-rigid first. Takes just a few minutes... look and feel of ureter v important for onward strategy. Allows easy insertion of WGW and assess whether access sheath appropriate. Never regret doing semi-rigid first; but can easily regret NOT doing it!!
On call this weekend - the view from the ground and again from the 14th floor…
But of course, it’s not what the hospital looks like, it’s what goes on inside that counts!!

If we’re honest, I suspect we’ve all had some struggles with this from time to time, especially after the challenges and pressures of the last 18 months.

It's a terrible term "poor historian". It's not the patient's job to give a good history; it's the doctors job to elicit it. "Challenging history" a better way of saying the doctor found it difficult to ascertain the information that was sought.
