Statins do not offer mortality benefit in those with a good triglyceride to HDL ratio (low trigs, high HDL). This is irrespective of LDL levels. While this paper is sure to spark debate, we (the authors) believe it to be sound. @LDLSkeptic @BenBikmanPhD

Just had a delightful consultation with a lady in her 70's who has published multiple books and articles. She stopped writing 8 years ago due to cognitive decline. 6 weeks after changing her diet, she is now working on her next book. Don't underestimate diet in dementia.

Low carb feeding of ventilated ICU patients reduced time on ventilator by an average of 62 hours. Other benefits included reduced respiratory rate, peak pressures and CO2 levels. This could reduce ventilator need by ~12% if similar results for COVID-19.

What does an endocrinologist do with a patient who has reversed diabetes, put ulcerative colitis into remission and ceased five medications for diabetes, blood pressure and cholesterol? She recommends he resume his old diet (on which he had all these issues), and take a statin.

Incredibly important paper in sports medicine. Most ACL injuries can be healed WITHOUT surgery. Traditionally, we immobilise the knee in extension, which pulls apart the torn ends of the ACL. Simply immobilising in flexion allows the ends remain close (and heal). Think different.

Only 1-2% of iron from legumes is actually absorbed. Despite this, many online articles recommend plant based sources of iron such as beans and lentils. How many people are iron deficient because they haven't been told the full story? Are these articles deliberately deceptive?

Ground breaking paper (BMJ Open Heart) exposes true findings of the publicly funded Womens Health RCT - following a low fat diet can be harmful. Misleading reporting of findings deceived most as to true findings- authors 'explanations' unscientific and may be ethically dubious.

In medical school, I was taught to prescribe statins to lower cholesterol 'as low as it goes' - basically drive it into the ground. This study found that patients on statins with the lowest cholesterol levels were more likely to die. Maybe I was taught wrong??

Don't think that fruit is natures candy.....you'll find this study interesting. 80 subjects were randomised to ≥4 or <2 servings fruits/day. After 6 months, the fruit rich group was fatter, had more liver fat, more liver damage, higher blood sugar and more insulin resistance.

This is an example of how science should work. The investigators hypothesised that dietary saturated fat leads to oxidation of LDL cholesterol. They found, in fact, that it actually polyunsaturated fats that have this effect, and reported it clearly. Say no to seed oils people.

The food industry is clearly gaming the food star system. A cereal containing 17% sugar gets 5 stars while smoked salmon (cured with salt), a nutritional powerhouse, gets 1.5 stars. No wonder so many people make bad food choices.

Consumption of monounsaturated fat from pork, lard and eggs was NOT associated with diabetes risk. Consumption of monounsaturated fat from plant foods (seed oils, fried bread, rice) WAS associated with increased diabetes risk. Could this be a 'causal' relationship??

Historic research found 73% of those with heart attack had abnormal blood glucose levels post OGTT. More modern research found only 23% of patients diagnosed with heart disease had 'high' LDL. And yet we forget about sugar and focus on LDL. Why?

Plant sterols (as found in seed oils) can be absorbed and incorporated into body tissues. This can interfere with the normal functions of cholesterol. This mice study found that plant sterol accumulation in the brain was 'virtually irreversible'. Is this cause for concern?

Blaming obesity on overeating is like blaming over drinking for alcoholism. It describes the problem (excess consumption), but doesn't explain the why. Insulin resistance has reliably been shown to precede obesity, and is a key part of understanding the why.

Greenhouse gas emissions per 100 k/cal are higher from fruit/vegetables than from meat/dairy. Furthermore, the nutrient density (ND6) of meat/dairy per greenhouse gas emissions is also lower than fruit/vegetables. Meat and dairy is good for you AND better for the environment.

This article from 1963 details how Merrell (drug company) misled authorities about severe side effects of the first cholesterol lowering drug to hit the market. Merrell hid these side effects by falsifying data - conduct which has been repeated many times since.

This patient on a very high protein diet, reduced her urinary protein to undetectable levels. Many doctors assume that high dietary protein causes high urinary protein (proteinuria - a sign of kidney disease). I have many patients whose kidneys improved on high protein diets.

Grass fed meat balls for $15 AUD per kg, or an industrially produced frankenfood (beyond beef) for $40 AUD per kg. Containing in order of quantity, water, pea protein and canola oil, along with numerous chemicals, beyond beef is neither environmental nor healthy.

Today I saw a super fit male in his 60's who was recommended a pacemaker due to a resting heart rate of 49. He consulted another cardiologist who thought the pacemaker was silly, but instead recommended a stent for a 70% stenosis (he was completely symptom free). Bad Medicine.

In August 2020, a young vegetarian was hospitalised with an inflammatory arthritis, unable to walk. Over the next few months, we changed her diet. Her CRP went from 73.8 to <0.4. She is now off all drugs. In the last 4 months, she has lost a further 9% fat and gained 3% muscle.

Magnesium lowers triglycerides and improves insulin sensitivity. Serum magnesium also inversely associated with coronary artery calcification. Meanwhile magnesium in our food supply is declining and deficiency is usually undiagnosed. Time to supplement? @FatEmperor @DaveKeto

New drug for Alzheimers approved by FDA after FDA's own scientific advisory panel advised against it. 10 out of 11 panel members concluded there was insufficient evidence this drug works. This is crazy when evidence shows ketogenic diets benefit both prevention and treatment.

A patient this week reported a large reduction in urinary tract infections after increasing her meat intake. This 1968 paper explains why. Urea (associated with protein intake) has an anti bacterial effect on urine. Also reduced antibacterial activity in dogs after removing meat.

A 1931 account of the 'almost exclusively vegetarian Akikuyu' and 'chiefly carnivorous Masai' tribes found adult Masai males were 5 inches taller, 23 pounds heavier and 50% stronger. The Akikuyu were more prone to bony deformity, cavities, anaemia, respiratory issues and ulcers.

Nurse: "It's OK, we're injecting him with insulin so he will be right". NO. It's not OK. When the cause of high blood sugar is the hospital food, the answer is to change the food. Not to inject drugs. Keto food dropped this man's BGL from 16 to 6-7 mmol/L (insulin stopped).

This most interesting paper from 1962 documents two cases of metastatic cancer (melanoma and cervical cancer) apparently successfully treated by insulin induced hypoglycaemia (used in the treatment of mental illness back then). Raises lots of questions yet to be answered.

More studies are questioning whether low salt diets are actually good for the heart. The SODIUM-HF trial found no benefit in those randomised to low salt diets, whilst analysis of TOPCAT trial data found strict salt restriction was associated with worse outcomes.

Gluten can have deleterious effects even in those without coeliac disease. Take these two double blinded randomised controlled crossover trials (basically the gold standard for studies) which found even small amounts of gluten in non coeliacs could cause / worsen depression.

Not all LDLs are atherogenic. It is more accurate to say LDLs are POTENTIALLY atherogenic. Many serve their functional purpose & return their lipid contents to the liver (indirect RCT). If LDL-P (apoB) is ⬆️, LDLs can invade intima where many do induce atherogenesis @society_eas

Fat acts as a sink for vitamin D, which is one reason why circulating vitamin D levels are lower with obesity. Weight loss can 'release' this sequestered (stored) vitamin D, increasing circulating levels. Higher vitamin D levels associate with improved metabolic health.

Vegetarians are at much greater risk of fractures. Is this due to their diet, or some other confounding factor? An RCT from 1999 suggests it is diet, finding that reversal of osteoporosis (yes REVERSAL) through nutritional means is dependent on a high protein intake.

Nutrient depletion of soil ought be a major concern of vegans and carnivores alike. Plants and grains (and animals consuming them) significantly nutritionally deficient compared to historic levels - need 4 x's the veggies to obtain the same level of some nutrients as in 1940's.

This fabulous study provided children with their own egg laying chicken. After 6 months, children provided with a chicken (and eggs) were 64% less likely to be anaemic and 57% less likely to suffer combined anaemia and stunting.

Many believe saturated fats increase LDL-C. This is incorrect. Rather plant sterols (as found in seed oils) inhibit cholesterol absorption, lowering LDL. Hence coconut oil (92% saturated fat / sterols) lowers LDL-C when compared with butter (51% saturated fat / NO sterols).

A vegan diet could reduce your intelligence. Nutrients of concern include: creatine, carnosine, taurine, omega 3, haeme iron, B12, D3, B6, selenium, folate and choline. Deficiencies especially concerning for developing brains.

Elevated HbA1c levels, (marker of high glucose) can identify indviduals with atherosclerotic disease even when considered low risk by traditional markers. This study found high glucose is a greater risk for heart disease than LDL levels. Don't blame fat for what the sugar did.

This prospective study of 63,550 subjects found that vegetarians had a 49% higher incidence of bowel cancer than non vegetarians. The results were statistically significant. This study may have more real world relevance than studies performed on religious communities.

Ingesting carbs during exercise was shown to reduce fat burning by more than half in this study of obese boys. Take home is that carbs can sabotage your exercise with respect to weight loss.

This study found watermelon may trigger headache attacks in ~30% of migraine patients. Other plant foods associated with migraines include passion fruit, oranges, pineapple, grapes, banana, cucumber and papaya.

Red meat does not putrefy in the gut. Meat is very well digested, with little if any residue after digestion. Insoluble fibre however is indigestable, feeding bacteria (putrefaction) producing gas and bloating. Faecal analysis is from a patient with severe pain and bloating.

To reduce risk of cancer mortality, it seems prudent to control fructose intake. The evidence that cancer is primarily a metabolic disease is accumulating. To pretend that nutrition is not important when it comes to cancer is negligent. @tnseyfried

Obesity shrinks the brain! Higher abdominal fat levels almost triple dementia risk. The brain is 2% of our body weight yet uses 20% of energy. This makes it vulnerable to metabolic disease. To look after your brain, you need to look after your metabolism.

I find this repulsive. In this study, investigators struggled to understand why patients who followed a low fat diet gained weight. So they implied the patients were lying rather than question their underlying (and incorrect) beliefs about low fat diets promoting weight loss.

Worried about dementia? Keto Diet (~65% fat) vs. American Heart Association Diet (~60% carb). KD improved metabolic profile (HbA1c, glucose, insulin, triglycerides and VLDL), cerebral perfusion and ketone body uptake. AHA diet did not. 10.1016/j.neurobiolaging.2019.09.015

10 years ago I published research that found conflicts of interest between the pharmaceutical industry and medical schools were not adequately controlled. I believe industry influence biases medical education at the expense of patients.

On average, vegans have poorer bone health than meat eaters as assessed by ultrasound. Twelve biomarkers strongly associated with bone health (11 postive, 1 negative) were identified. Most, if not all of these are impacted deleteriously by nutrient deficient vegan diets.

Uptake of glucose can be up to 43 times greater in cancer cells. Is it any wonder that obesity and diabetes are linked to at least 13 different types of cancer including pancreatic, liver, colon, breast, endometrial, and bladder cancers. @tnseyfried

Patients with heart disease are often told to avoid sodium (salt). This just published study randomised 806 subjects to either a low sodium or their usual diet. There was a non statistically significant trend (HR 1.38, p=0.32) to early death in the low sodium group.

The worlds biggest study on low fat diets has been described as a failed experiment. The truth is, it only failed to find what the researchers wanted. When they discovered that low fat diets could be harmful, they went out of their way to hide the truth.
