@UKGastroDr Unfortunately I've not come across the same courtesy when I have been too a and e as a user. Even got told off by one triage nurse that my 'nhs badge' won't get me anywhere with her or in this ED. Where as I always make it a point to see colleagues/ staff quicker.

As SAS we hold the bleep for our on call trainee colleagues so that they are able to.

Of course there are ways to progress, but significantly harder if not in training

A junior Colleague asked me - how are you pursuing a post CCT level fellowship aged 32. I replied - I came to UK in 2019 - 1 year SHO level job and 3 year reg jobs later - completed MCH with dissertation, MRCS an FRCS TrOrth before applying. And I don't have hobbies anymore 😭

@MedCrisis @scullingmonkey You're both right, I don't agree with it. I myself am working as a fellow on this contract, as this is what I was offered. On call provision adds some banding - varies on frequency and odd working hours. Perhaps this is why I'm just not surprised enough looking at the advert :)

It is a post CCT level post FRCS fellowship. I have shared irritation & awaiting a reply

I have been a BMA member 5 years and switched 4 jobs. BMA if anything has never really helped me at all. Including when I was overpaid 10k by a trust and sent a bill of 20k in error. Took me many moons to get it resolved & BMA sadly were only mildly helpful.

@EMManchester @MStott88 It took me half a day to get a much needed and indicated MRI scan in new delhi, followed by specialist consult same day, where in still waiting for appointments for months, let alone MRI being requested!

And then I have had to chase HR to recalculate - which they eventually do after months of negotiation and this has varied in trusts. There is no set principle.

@hhjhammond I don't think telephone consults work very well. Personally not a fan of those... If the wait times were better they might make more sense, but a telephone call in 4 months , and if not resolved another appointment in further more weeks - seems like a waste to me

@emmdechen Wonder when was the last time people showed such support and concern for LED doctors, SAS colleagues. This is a systemic devaluation of doctors. And most senior doctors and policy makers are complicit in systemic devaluation of their own profession.

@rahttled_doc @NHSEngland @NHSScotland I worked at North east at @CDDFTNHS for less than a year in ortho. Left job due to so many issues with the work environment as a "non trainee" registrar. Never had an exit interview. One fine day over a year after I left I got a call from HR asking for feedback.

However in my experience BMA has not been particularly helpful in supporting Me either as LED or SD. Even as a SD, I was told in no clearer terms by BMA that counting my experience is totally dependent on HR and completely their discretion.

@JoanneYounge @sas_collective I did thank you - because it is post CCT level fellowship as I am post FRCS - however I am now back on MN37 pay scale - with a cap on remuneration, since I am now an LED - with even lesser rights 😅 but yes the department I work in and consultants I work with are awesome :)

@RobJimFleming There is a massively negative connotation and intonation when this term is used universally in every trust I have worked. Often they are synonymous with "non trainee" and most often used by consultant body. most job adverts for specialty doctor say - looking for "middle grade"

@fatima_medic This is just terrible work ethic. Most calls to request are generally made after discussing with reg/consultant anyway. duty radiologist can simply verify by asking if this has been discussed. Refusal to take a call based on rank is is just unprofessional IMO.

We can try to make people aware and as you rightly put it - talk about "parity of esteem". This division exists not only for voting rights etc, but for fellowships, career progression

However, contract states a min 4 yr work ex post fy years is essential In that specialty before eligibility for SD contract. 2 trusts in past have taken away 4 years of my experience while calculating my base pay on mc75 for the same reason.

Some waiting times for specialist clinics in the #NHS are 18 months. What is the point of that. By that time, either the condition will be gone, or the patient will have suffered for a year and a half, or they might even be dead. That is no service at all.

@DrSanjay277 Specialist needed. However the downside Is lack of team based MDT care. As long as patient belongs to a doctor and not a practice - MDT patient centric approach will be less likely - however I do think niche knowledge is needed to cope up with ever growing research #MedTwitter

Answerable to managers and consultants - focus being on numbers rather than quality of care. At some point doctors not in training will also stop caring and going above and beyond and do just the bare minimum needed - ie, service provision by hours. #SaveTheNHS

@RobJimFleming @Lamboozler These exams massively disadvantage people in non training posts. It took me two attempts at MRCS. To get signed off for FRCS - I was completely at the mercy of the consultants in my department. One boss in previous employment told me my previous overseas training counts for (1)
