Time for a change! I've just started my dream clinical academic job in global child & youth mental health with
@childmindinst
where I get to work with
@DrS_Reynolds
& local partners to culturally adapt evidence based interventions in contexts inc Brazil and South Africa 1/3
As professionals, we must get better at detecting autism in girls at an earlier age. Take for example special interests which often get missed as they do not fit the (male) stereotypes. Here are just some examples of special interests I've seen in autistic girls under 11 yrs:...
Where do all the children & young people with moderate-level severity mental health problems go for support?
"Not severe enough" for CAMHS, yet "too complex" for our school based Mental Health Support Teams🤷♀️
So many children are falling into this gap in service provision😞
...Animals, gymnastics, dance, nail art, hair care, stage makeup, horses, reading, crystals, gems, shells, plushies, stickers, Hello Kitty, drawing, mosaics, jewellery making, sculpture, Lego, Minecraft, Roblox etc. Autistic girls' special interests are as diverse as they are!..
@flavaadit
@janinemas0n
Ok I've watched the whole thing and it's brilliant but so confusing! Who is in the right? Can a clerk remove people? How come the first chair was posing as a clerk? How can someone be in trouble for missing 4 meetings that didn't take place?! Local govt procedures are baffling!
As one step towards improving earlier diagnosis, we need to challenge stereotypes of what we think autism looks like and and listen to autistic girls. If you want to learn more about why autism can be missed in girls, I recommend
@AutisticGirls_
report:
CAMHS referral rejections:
"It's not mental health, it's neurodevelopmental"
"It's not mental health, it's behavioural"
"It's not mental health, it's social care"
The exclusion of certain groups by the narrow CAMHS definition of mental health seems to me to be a rights issue 1/3

I used to work in CAMHS & have great respect for colleagues still there but I think it is important to name the structural issues which are in part being driven by commissioning decisions. "We're not commissioned to treat neurodevelopmental conditions" is not good enough in 2024.

It is gutting to hear again and again of parents who are unable to get help for children in distress. Today I was told that one NHS

Jonah Hill's film about his therapist
#Stutz
is extraordinary. You will think for the first 20 minutes that it is one kind of documentary, then it flips. The rawness, the emotional intimacy, the tension between authenticity & inauthenticity. Seriously, do watch it.
Jonah Hill sits down with his psychiatrist Phil Stutz for an unorthodox session that flips their typical doctor-patient dynamic, bringing Stutz's Tools to life in a humorous, vulnerable and ultimately therapeutic experience. Stutz is now on Netflix

Absolutely spot on- the widespread use of term 'mental health' is almost meaningless
While seeking to destigmatise mental disorders, it has led to marginalisation & more discrimination of people with conditions like psychosis/schizophrenia.

This couldn't be any more British. Couldn't park anywhere near the hospital so was late and then "had a rather nasty lunch" but given the most amazing free treatment. How very NHS.
Some personal news, I'm moving across from the NHS to work in applied child & youth mental health research (details to follow). Feeling so grateful to have been part of a paediatrics team that does so much for vulnerable children. Sad to leave but excited for new opportunities💙
I get so angry when I see kids with multiple adverse childhood experiences being denied access to our mental health services
Charities are often left to pick up the pieces but limited in what they can offer.
I'm keen to connect with others who are trying to bring about change✊
"Trauma informed training" is being rolled out in our schools with poor scientific evidence. My colleagues and I are very concerned about the potential for harm from these programmes. Anyone interested in this area, do check out papers from
@lfoulkesy
and Sebastian Monteux.
It really concerns me that my schoolteacher friend just had 3hrs "trauma informed training" in which they were told "diagnosis is nearly always unhelpful", "antidepressants don't work", neurodivergence is a result of trauma etc. Also no chance to ask Qs. Sounds like preaching.
Sad to see some negative tweets about

The division of society into a two-tier system of the "healthy" and "those with pre existing medical conditions" is something you would encounter in a dystopian science fiction novel.
Except this is our new reality. And there is no sign of this toxic narrative fading away.
I work in a medical speciality where we are trying to grow psychology. I have *huge* respect for my medical colleagues. So it was disappointing to receive an email from a doctor telling us they consider clinical psychologists "more of hinderance than help"😩

Can't help but feel frustrated when I see £££ for mental health research that lacks any real world implementation, when in paediatrics we are seeing so many children & families struggling and have no services to refer them to. We need £££ on the frontline now, not down the line.
Essential thread from
@naomicfisher
for anyone working with children👇
I hear similar things from the parents we see in our NHS neurodevelopmental assessment clinics. Children and families suffer when our educational systems are so inflexible.
When I tell people that I work with children and families who have problems at school, they often nod and look sympathetic. 'Bullying is terrible', they say. Yes. It is, but it's not bullying I hear about most. Here's what families tell me. (with

@OK_Magazine
Please reconsider your choice of photographs in this piece. These are images of a person who was very unwell with a serious eating disorder- is this how she or her family would want her to be remembered? Also seeing images like these can be very difficult for people with EDs.
Ok so now we have health professionals recording videos straight off a shift with lengthy details of a patient they just treated. That "case" is a person. They know who you are. They can identify themselves from this video🤦
Patients are not content.

As a clinician and mental health researcher, I can't accurately capture my thoughts on this complex phenomenon in a tweet/Insta post/reel/TikTok video - and that, dear readers, is part of the problem.
Mental health providers are seeing an uptick in teens diagnosing themselves with mental illnesses — including rare disorders — after learning about the conditions online, particularly on TikTok. Some are embracing ineffective or inappropriate treatments.

We've got to a bizarre stage with the trauma informed movement where some of its own proponents have become non-trauma informed🤦♀️
Yet it's very hard for qualified clinicians to engage in meaningful dialogue with content like this on platforms that have no interest in nuance.
don’t usually get drawn into this but actually had to check if this was a spoof… There is no benefit in such extreme black and white messages … never met a mental health clinician who is like”yes fully endorse chemical restraints for oppressed individuals… anymore questions?”

Lack of clinical academics threatens NHS research and teaching, “The foundations [of university medicine] are starting to be eroded,” she said. “The pipeline for clinical academics is really looking quite frightening . . . & it is not being talked about.”
Me, a clinical psychologist: numbers of social media followers have absolutely no meaning and no impact on my sense of self-worth.
Also me: ooh who's going to be my 2,000th follower?!
Plot twist. I have just had a paper accepted in its third iteration after extensive comments from a very persistent Reviewer 2 who actually (whisper it) made the paper much better.

Good to see the author flagging that staff outside HDU are also caring for Covid+ patients but with only Level 1 PPE. Hard to believe we are this far into the pandemic and inadequate PPE is still an issue. Colleagues on general wards are doing amazing work but are scared.
This is fascinating, the 2nd large RCT to be published in a week(!) to find that a universal school-based intervention did not work and even had potential iatrogenic effects.
I would love to hear people's thoughts: how should we be doing early intervention in CYP mental health?
Out now in Psychological Medicine, we report the effects of a large RCT of a universal school-based CBT informed intervention for adolescent mental health.
Despite increasing m/h knowledge, we found null, and some small iatrogenic effects, a🧵

This to me is one of the dilemmas of being a mental health professional on social media:
Do we call out blatant misinformation and bad takes, and so risk feeding into the controversy that the original poster was hoping for, thus boosting their reach?
Or not engage?
CBT Therapists, it is important that we are aware of how CBT is talked about (and misrepresented) online.
This is why discussing what CBT is + what it isn't is a vital part of the contracting process. To borrow from Helen Kennerley: "CBT is on trial and I'm on trial, not you".
This is a good encapsulation of why CBT -- the worst and most popular form of therapy -- is completely useless.
Every one of our behaviors is there for a reason. It lifts some weight from us, it helps us avoid something hard or scary in our pasts, it serves some purpose to keep
